To take this survey you will need a code number that will act as your login and password. Your code should have been given to you by your employer. Enter your 10 digit code in both the Login and Password fields. The example below is just a sample. The easiest way to do it correctly is to copy and paste your code into the desired fields.

Please do not take the survey more than once. If you start the survey and are unable to finish it, close your browser window without hitting the "submit" button. Your employer has paid for a finite number of surveys. If you take the survey more than once it could prevent your results or someone elses results from being tabulated. All surveys are confidential . Your responses can not be traced back to you as an individual, so please be honest.
